Non-Hormonal Treatment Options
Non-hormonal approaches are appropriate for many women — including those who prefer not to use hormones, those with certain contraindications, or as part of a comprehensive plan alongside hormonal therapy. Lifestyle modifications are foundational for every woman going through the menopause transition.
Lifestyle Medicine
Lifestyle interventions are not a consolation prize — they are powerful, evidence-based tools that can meaningfully reduce symptom severity and protect long-term health. They form the foundation of care for every patient at Metis Midlife Medicine, regardless of what else is recommended.
Nutrition
A whole-food, protein-rich, lower-glycemic diet supports insulin sensitivity, body composition, bone health, and cardiovascular risk reduction. Phytoestrogen-rich foods (soy, flaxseed) may offer modest vasomotor benefit for some women.
Resistance Training
Strength training is essential in menopause — it preserves muscle mass (sarcopenia prevention), protects bone density, improves insulin sensitivity, supports mood, and favorably affects body composition.
Cardiovascular Exercise
Aerobic activity improves cardiovascular health, reduces vasomotor symptoms, supports mood and sleep, and helps manage weight. Aim for at least 150 minutes of moderate intensity per week.
Sleep Hygiene
Consistent sleep and wake times, limiting caffeine and alcohol, cool sleeping environments, and wind-down routines can meaningfully improve sleep quality disrupted by hormonal changes and night sweats.
Stress & Mind-Body
Mindfulness-based stress reduction (MBSR), cognitive behavioral therapy (CBT), and yoga have demonstrated benefit for vasomotor symptoms, mood, and sleep in clinical trials. CBT for menopause is guideline-supported.
Alcohol & Smoking
Alcohol disrupts sleep, triggers hot flashes, raises breast cancer risk, and negatively affects bone density. Smoking accelerates menopause onset and worsens vasomotor symptoms. Cessation is strongly recommended.
Neurokinin Receptor Antagonists
This class works by blocking neurokinin B (NKB) signaling in the hypothalamus — the pathway that becomes dysregulated when estrogen declines — to directly reduce the frequency and severity of hot flashes and night sweats. Fezolinetant (Veozah) was the first FDA-approved non-hormonal medication specifically designed for moderate-to-severe vasomotor symptoms. Elinzanetant (Lynkuet) is a more recently approved dual NK1/NK3 receptor antagonist that also targets the NK1 pathway, offering additional benefit for sleep disturbance alongside vasomotor symptom relief.
SSRIs & SNRIs
Low-dose paroxetine (Brisdelle, 7.5 mg) is the only FDA-approved SSRI for vasomotor symptoms. Other SSRIs and SNRIs — including escitalopram, venlafaxine, and desvenlafaxine — are commonly used off-label and have demonstrated efficacy for hot flash reduction in clinical trials. These medications may also address the mood and anxiety symptoms that frequently accompany the menopause transition.
Gabapentin
Gabapentin is an anticonvulsant that is used off-label for vasomotor symptoms, particularly night sweats and sleep disturbance. It is especially useful when hot flashes are most disruptive at night. Sedating effects can be a benefit for women with significant sleep disruption, though daytime dosing may cause drowsiness.
Oxybutynin
Oxybutynin is an anticholinergic medication primarily indicated for overactive bladder, but it has been shown in clinical trials to reduce vasomotor symptom frequency and severity. It offers a non-hormonal option particularly suited to women who have both hot flashes and urinary urgency or urge incontinence.
Non-hormonal vaginal moisturizers (such as Replens, Revaree/hyaluronic acid-based products) are used regularly — typically every 2–3 days — to restore and maintain vaginal moisture and pH. They differ from lubricants, which are used situationally. Moisturizers can meaningfully reduce vaginal dryness, discomfort, and urinary irritation. They are a first-line option for women who are not candidates for or do not wish to use vaginal estrogen.
Ospemifene is an oral selective estrogen receptor modulator (SERM) FDA-approved for moderate-to-severe dyspareunia (painful intercourse) and vaginal dryness due to menopause. It acts as an estrogen agonist in vaginal tissue — improving vaginal cell maturation and moisture — while acting neutrally or as an antagonist in breast tissue. It is a compelling option for women with significant genitourinary symptoms who prefer an oral, non-estrogen approach.
Hormonal Treatment Options (MHT)
Menopausal Hormone Therapy: What the Evidence Actually Shows
For decades, fears stemming from a misinterpreted 2002 study led to dramatic underuse of hormone therapy. Current evidence — and updated Menopause Society guidelines — is clear: for healthy women within 10 years of menopause, hormone therapy is safe and effective, and may offer significant long-term health benefits including cardiovascular, bone, and cognitive protection. The risks depend on the type, dose, timing, and route of therapy — and are highly individualized.
Estrogen therapy is the most effective treatment for vasomotor symptoms (hot flashes, night sweats) and addresses the full spectrum of menopause symptoms. Systemic estrogen also provides protection against bone loss and may benefit cardiovascular and cognitive health when initiated in the early menopause window ("timing hypothesis"). Women with an intact uterus must take a progestogen alongside systemic estrogen to protect the uterine lining.
Transdermal Patch
Applied to skin 1–2× per week. Bypasses liver metabolism, associated with lower clot risk than oral estrogen. Consistent delivery of estrogen.
Transdermal Gel or Spray
Daily application to skin. Flexible dosing; particularly convenient. Like patches, avoids first-pass liver metabolism.
Oral Estrogen
Taken daily by mouth. Effective and well-studied, but undergoes first-pass hepatic metabolism, which may slightly increase clot and triglyceride risk compared to transdermal routes.
Vaginal Ring (Systemic)
Femring® releases systemic doses of estrogen. Inserted every 90 days; convenient for women who prefer a less frequent regimen.
Progesterone and synthetic progestins are prescribed alongside systemic estrogen in women who have a uterus, to prevent endometrial hyperplasia and cancer. The type of progestogen matters: micronized progesterone (bioidentical) has a more favorable safety and tolerability profile than many synthetic progestins, including potential sleep-promoting effects and a lower impact on breast cancer risk compared to synthetic progestins such as medroxyprogesterone acetate (MPA).
Micronized Progesterone (Prometrium®)
Bioidentical oral capsule taken daily or cyclically. Associated with better tolerability, sleep support, and favorable cardiovascular and breast profiles compared to synthetic progestins.
Progesterone Vaginal Cream/Gel
Topical or vaginally applied; may reduce systemic side effects. Used in select clinical contexts.
Levonorgestrel IUD (Mirena®)
A highly effective option for endometrial protection in women who use systemic estrogen. Provides local progestogen with minimal systemic absorption — and may reduce heavy perimenopausal bleeding.
Synthetic Progestins
Medroxyprogesterone acetate (MPA), norethindrone, and others are effective for endometrial protection and are found in many combination HRT products. Prescribing is individualized based on history and preference.
Several FDA-approved products combine estrogen and progestogen in a single delivery system — simplifying regimens for women with a uterus.
Combination Transdermal Patches
CombiPatch® and Climara Pro® deliver both estrogen and progestin transdermally. Applied 1–2× per week depending on the product.
Combination Oral Tablets
Products such as Bijuva® (estradiol + progesterone) and others offer a single daily pill. Convenient and well-studied.
Low-dose vaginal estrogen is a highly effective treatment specifically targeting Genitourinary Syndrome of Menopause (GSM) — vaginal dryness, atrophy, painful intercourse, and urinary symptoms. Because it is applied locally at very low doses, systemic absorption is minimal, and it is generally considered safe even for women with breast cancer history (in consultation with their oncologist). Women using vaginal estrogen alone do not typically require a progestogen for uterine protection.
Vaginal Estrogen Cream
Estrace® cream or Premarin® cream applied with an applicator. Flexible dosing; very effective for vaginal atrophy and urinary symptoms. Can also be applied to the vulva.
Vaginal Tablets / Inserts
Vagifem® / Yuvafem® (estradiol inserts) are inserted with a small applicator. Minimal mess; twice weekly after initial loading. An excellent first-line local option.
Vaginal Ring (Local)
Estring® releases a very low, local dose of estradiol continuously over 90 days. Inserted by the patient or provider; low-maintenance and highly tolerable.
Prasterone (brand name Intrarosa) is an FDA-approved intravaginal DHEA suppository for moderate-to-severe dyspareunia (painful intercourse) due to menopause. DHEA is a precursor hormone converted locally in vaginal cells into both estrogen and androgens, restoring tissue without significant systemic hormonal exposure. It is a compelling option for women with painful intercourse who prefer not to use vaginal estrogen or who want a non-estrogen alternative.
Intravaginal Insert
Inserted nightly at bedtime. Acts locally at vaginal tissue. FDA-approved; does not require a progestogen for endometrial protection.
Testosterone levels decline with age and the menopause transition, and low testosterone in women is associated with reduced libido, fatigue, and diminished sense of vitality. Testosterone therapy — using low-dose topical formulations — is supported by the Menopause Society and the Global Consensus Statement for treatment of hypoactive sexual desire disorder (HSDD) in postmenopausal women. In the United States, testosterone is not currently FDA-approved for use in women, so prescribing involves off-label use at female-appropriate doses, with careful monitoring.
Topical Testosterone Cream or Gel
Applied to skin daily in very small, female-appropriate doses. Most commonly used approach; allows flexible titration. Compounded or diluted from commercial male products.
