This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Metis Midlife Medicine, PLLC is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI.

Pending attorney review — draft only

This is a placeholder Notice of Privacy Practices. A complete, HIPAA-compliant NPP must be drafted and reviewed by legal counsel prior to seeing patients. The final NPP will address all required elements under 45 CFR § 164.520, including specific uses and disclosures, patient rights, and complaint procedures.

How We May Use and Share Your Health Information

We use and share your health information in the following ways:

Treatment — To provide, coordinate, and manage your medical care, including sharing information with other treating physicians or specialists as needed
Payment — To process payments for services rendered (Metis is a cash-pay practice; we do not bill insurance directly)
Healthcare operations — For quality improvement, training, and administrative functions related to running our practice
As required by law — When required by federal, state, or local law, including public health reporting, court orders, or law enforcement requests

We will not use or share your health information for other purposes without your written authorization, except as described in this notice or as otherwise permitted by law.

Right to an accounting — You may request a list of certain disclosures we have made of your health information
Right to restrict — You may request restrictions on how we use or share your information

Effective Date

This Notice of Privacy Practices is effective as of the date shown above. We reserve the right to change this notice and to make the new notice effective for all health information we maintain. Current copies will be available on our website and upon request.
